If you apply for coverage after your enrollment deadline, or if you choose coverage over the guarantee issue amount, you will need to complete a medical questionnaire (Evidence of Insurability Form) which you can obtain from the above section (#1 Review Your Plan Details). You may also be required to take certain medical tests at Unum's expense.El Paso County only allows changes to coverage during the plan year for those that qualify due to a family status change.
